• Immunization and Health Requirements

    NC Health Assessment Information

    North Carolina law requires every student entering public schools in N.C. for the first time to receive a health assessment. The assessment must occur within 12 months prior to entering school. The medical provider, parent or guardian must provide a completed North Carolina Health Assessment Transmittal Form to the principal of the school on or before the student's first day of attendance. 

    Students are required to be compliant within 30 calendar days from their first day of attendance.

     

    NC Immunization Information

    North Carolina law requires all children in the state to receive certain immunizations.

    School principals are responsible for enforcing state immunization laws for school entry.

    The school must notify the parent(s), guardian or person in loco parentis that they have 30 calendar days from the first day of attendance to present the required up-to-date immunization record for the student. If the student's immunizations are not up-to-date, the required immunizations must be obtained within the same 30-day period.

    At the end of the 30 calendar day period, any student without a Certificate of Immunization showing that the student has received the required vaccines shall be prohibited from attending school until a Certificate of Immunization as required by law is provided or shows that they have begun the immunization process.

    Students are required to be compliant within 30 calendar days from their first day of attendance.
