BYOD  Student/Parent Agreement

• Each parent and child are expected to read, discuss and adhere to the items listed in the Student/Parent agreement.
• Both must initial EACH box on the form. Both parent and child must sign the form.
• Any violation of these policies may result in removal of BYOD privileges and technology use suspension.
• If this form has been previously completed then it does not need to be signed again.
