    Please respond to this form if you have a concern with your schedule that you would like a counselor to review. Please note that elective changes do not fall into the criteria listed below. If you are scheduled into an elective that you requested as a primary or alternate selection, your request will be denied. 

    Approval will ONLY be granted for the following:
    - Missing a class "hole" in your schedule
    - Level changes to increase in rigor. Ex; Math 2 to Math 2 honors. **based on space availability 
    - You have not met the prerequisites for the class you are currently scheduled for
    - You need to repeat class that you previously failed
     
    DO NOT put Early Release/Late arrival requests on this document.  If you are a senior requesting ER/LA, you must fill out this form and turn it in to student services. Please note, we can only grant LA/ER if there is room to make the necessary adjustments in your schedule. Grades 10-11 CCP Early Release use this form.

    This document will close on August 20th to prepare for the first day of school. The form will reopen on August 25th and close on August 29th. Please do not email your counselor as this form is the only way to initiate the request. If approved, the adjustment will be reflected in Infinite Campus. If denied, we will send an email response to your student email address.
