Kindergarten Health Assessment

 

Hello Kindergarten Families,

 

I wanted to connect with each of you regarding the Health Assessment and Immunization Reporting requirements for all Kindergarten students.

 

North Carolina General Statutes (G.S. 130A-155) requires immunizations for every child attending a public school in the state.  Every parent or guardian is responsible for ensuring that their child receives required immunizations.

 

The North Carolina General Statutes (G.S. 130A-440) also require a Kindergarten Health Assessment for every child attending public school in this state.  The assessment must have been completed within 12 months prior to school entry.

 

All immunization requirements and health assessments are now due by September 30, 2020.  Please contact your primary care provider or local health department to schedule an appointment for the appropriate immunizations and/or Kindergarten Health Assessment if you have not completed this requirement.  Students will be granted a 30-day grace period which expires on October 30, 2020. This means students will be excluded from school (in-person or virtually) on October 31, 2020. 

 

If your child has a medical or religious exemption, you are required to provide proper documentation. If this circumstance applies to your child, and an outbreak of disease occurs, your child will be excluded from school until the disease is no longer contagious.  This protects the health of your child and prevents the spread of disease.  If you have additional questions, please contact the school at 919.694.8770.
