Families wishing to request a different track assignment for the 2025-26 school year must complete this online form by 5:00 pm on Monday, February 3, 2025. Forms received after that time will not be considered.

If you are requesting a track change for multiple siblings who will attend Salem Middle School, you must complete a separate form for each child. If you are also requesting a track change for a student at a year-round elementary school, you must ALSO submit a form to the elementary school. If you do not submit forms at both schools, we cannot guarantee that both students will be moved to the same track. Contact the elementary school for details on their track change process.
