Summer DEC Forms

Summer 2024 Process for Obtaining a Driver's Eligibility Certificate

How do I obtain a DEC during the summer?

  • DECs will be issued by the Front Office during the summer months as the Student Services office is closed.
  • DECs can be picked up Monday through Friday from 8am - 3pm (except holidays).
  • No appointments are necessary.  Bring your child's Driver's Education Completion Certificate and proof of address when you come in even if you have previously emailed it.  Ms. Connors is not at AFHS in July and the front office is not able to see any previously sent information.

Important Things to Know

  • DECs are only valid for 30 days.
  • A parent / guardian OR student may obtain the DEC form.  Please follow the instructions below.
  • Per NCDMV rules, staff will check the student's grades for adequate progress before issuing the DEC.  Grades are reviewed by semester.  Students MUST pass a minimum of 70% of classes the semester prior in order to receive a DEC.
  • To save time, it is best to schedule an appointment with the DMV prior to receiving the DEC from the school (again, the DEC is only valid for 30 days).

Bring the following documents with you when you come to school:

  • Completed Driver's Education Certificate (even if it has been emailed previously)
  • Proof of address (must match what is in PowerSchool).  Proof of address must be one of the following:
    • Current water, gas, or electric bill in parent's name that is no more than 30 days old
    • Newly signed lease that is no more than 60 days old
    • settlement statement, or
    • Offer to purchase agreement, in limited circumstances.

Important Note:  If your student picks up the DEC, please ensure that the parent / guardian reads, signs, and dates the DEC prior to arriving at the DMV.  The DEC will expire 30 days from the date that AFHS includes on the certificate, not the date the parent signs it..  Please do not obtain a DEC more than 30 days prior to going to the DMV.
