If you wish to request a track change for the 2024-25 academic year, you may use this electronic form to submit your request or you may complete a paper form available in the front office at the school. Requests will not be prioritized on a first come, first served basis. Due to current enrollment, there are limited opportunities for changes on any track. If a track change is made based on this request, the new assignment will be the final assignment and cannot be changed until the track change request process opens up for the following year.
The parent/custodian submitting the request acknowledges that both parents/custodians of this student are in agreement with this request being submitted. In cases where one parent/custodian has sole custody of the student, the person submitting this request acknowledges his/her legal authority to do so.
