Transportation Service Request

The Transportation Service Request Form is used to appeal a bus stop location or route. All route and stop appeals from parents should be submitted in writing to the transportation manager. The appeal should state clearly the action which is desired and justification for the requests. The response to any appeal shall also be in writing with answers clearly stated and fully explained.

Drivers' Awareness Record (Medical/Physical)

The Drivers' Awareness Record is available to be used to complete pertinent medical and/or physical information helpful to the driver. The information required can be completed by a classroom teacher or a parent and should be done only for those students with special medical or physical needs. The form will be readily accessible in the bus or contract vehicle should there be an emergency and will not be removed from the vehicle. Identified students who transfer to another bus or school should have their form sent to the receiving driver or school.
