Form 1702 - Parent Request and Physician's Order Form for Medication (Español, PDF)
If your child will require medication(s) at school, you will need to provide a completed Form 1702 "Parent Request and Physician's Order Form for Medication" by you and your child's doctor (download links at right).
Form 1702 is also available at your child's school .
At the beginning of each school year, an adult will need to:
- Bring the medicine(s) along with the completed Form 1702(s) to the school
- Medicine(s) can not be given until a completed Form 1702 is received
- In the event of an emergency if no medicine(s) is on hand 911 will be called.
