Who is eligible for dental insurance?

Employee must be full-time permanent to qualify.
Reliance Standard is the dental plan provider.
Dental insurance cannot be effective in the first month of employment.
Coverage must begin the first day of the second month following your month of employment to eliminate waiting period for fillings (6 mo.) and crowns (24 mo.), etc. There is not a waiting period for cleanings.
Premiums are deducted a month in advance. Premiums are prorated based on your months of employment. The exception is year-round school employees.
Premium rates are calculated as follows:
10 month employees traditional school 10 month rate
10 month employees traditional school electing 12 month pay 10 month rate
10 month employees year round school 12 month rate
11 month employees traditional school 11 month rate
11 month employees traditional school electing 12 month pay 11 month rate
11 month employee year round school 12 month rate
12 month employee 12 month rate
Eligible employees are to complete the dental form either to enroll or decline coverage.

Dental insurance is cancelled the first day of the month following date of termination during a school year.
A dental book is located in the new employee packet for information. You must complete enrollment form for coverage.
