ENROLLMENT: What is a Discipline Status Form?

A Discipline Status Form is required by Wake County for all new students and for students transferring into WCPSS from other school systems. The form should be given to you when you register your child for enrollment. The form must be completed and returned to the assigned school before enrollment.
Please remember that a notarized signature of the parent or legal guardian is required.
