How can I arrange a special diet to accomodate a medical condition for my student?

You may print a diet order form from the link below or obtain one from the Child Nutrition Manager at your school. Take the form to your physician for completion. Be sure that all information is accurate and complete. Return the form to the Child Nutrition Manager at your school who will forward a copy to the Registered Dietitian. The manager will work with our Dietitian to make the necessary menu modifications.
Menu adjustments are made only for diagnosed medical conditions. We are unable to honor food preferences. We do, however, offer a variety of additional menu choices and a la carte items to accomodate individual likes and dislikes.
