Special Diets
Child Nutrition Services can make accomodations for special diets as ordered by your child's healthcare provider. You may print the Diet Order Form below or obtain a copy from the Child Nutrition Manager at your school. Diet Order Forms must be completed annually.
Follow the instructions on Page 2 of the Diet Order Form to ensure the information is accurate and complete. Part II must be completed by your physician.
Return the form to the Child Nutrition Manager at your school who will forward a copy to CNS's Registered Dietitian. The manager will work with our Dietitian to make the necessary menu modifications.
Menu adjustments are made only for diagnosed medical conditions. We are unable to honor food preferences. We do, however, offer a variety of additional menu choices and a la carte items to accomodate individual likes and dislikes.
